How many sections are there to the Personalised Report?

There are 6 sections to the personalised report. They are the following:

1. About your weight
2. About your health and lifestyle
3. About your eating (1) What are your eating patterns and food intake like at the moment?
4. About you eating (2) What influences your eating
5. About your physical activity
6. About your body image

How long does each section take to do?

Each section takes about 10 minutes to do.

How much does it cost?

The Personalised Report costs £4.50 to do on-line. This includes all 6 sections. You can pay by either credit card or cheque. If you pay by credit card then you will be able to do the report right away. However, if you pay by cheque you will need to wait until we receive your cheque. Once we do, you will be notified via e-mail and then you will be able to log on and do it.

Yes.

When do I get the report?

You get the report right away. Just fill out the questions and hit the submit button. You can then print it out for your own records. You will also be able to go back to the system and view your report at any time.

Do I have to do all 6 sections at the same time?

No. When you sign up, you create an account with a password which allows you to log on and off at any time. You can then complete one or more sections of the report, and go back to uncompleted sections at a later date; this could be a month or even a year later!

When you do decide to complete the questionnaire, simply click 'Enter/View Reports' on the main menu and enter your email address and password.

Do I have to do the Personalised Report in order to use the Shape-Up workbook?

It is not essential that you do the personalised report and many may choose to do the Shape-Up programme without doing a report first.

Can I complete a personalised report without buying a copy of the Shape-Up workbook?

Yes - lots of people only want an assessment done. They may choose to come back to the workbook at a later stage.

Can I do the Personalised report more than once?

Your payment of £4.50 will allow you to do one Personalised Report. You can choose to do as many reports as you would like, but you would be charged for each further one you do.

Can I be sure that my personal details and report will remain confidential?

Any information you provide will be kept strictly confidential and will not be passed onto any other groups or individuals. The system set up is secure.

What if I would rather fill in a paper version of the questionnaire - is this possible?

We can send you a paper version of the Shape-Up questionnaire for you to fill, just contact the Weight Concern team. You may also download the report questions as PDFs to print out by clicking here. We would then need you to send your completed report back to us, so that we can analyse the results and produce your personalised report. Unfortunately, we have to charge more for this service to cover the staff time to produce your report (£16.50 per report analysed).
